Rejection Criteria

9.1 First level rejection criteria - samples shall not be accepted or processed if any of the following criteria exist:

  1. No name on the request form / specimen.
  2. Requesting hospital / unit / ward / clinic not identified.
  3. Incomplete identification card / registration number.
  4. No name, signature and stamp of requesting doctors.
  5. No clinical history information.
  6. Information in request form and sample container is different.
  7. Mislabeling of sample / request form.
  8. Unsatisfactory specimen when received, for example spillages, breakages and etc.
  9. No hazardous sticker for infectious disease / sample.

9.2 Second level rejection criteria – sample not suitable to process if the following criteria exist:

  1. Inappropriate sample container / request form.
  2. Blood is haemolysed / clotted.
  3. Insufficient sample.
  4. Aged sample.
  5. Sample is not suitable for analysis.
  6. Test is not available.
  7. Inappropriate sample transportation.
