Acceptance Criteria for Urgent/Verbal Request (Chem Path)

ACCEPTANCE CRITERIA FOR URGENT REQUESTS

1.  Request shall be made via phone by authorized personnel.
2.  Request form must be stamped URGENT where applicable.
3.  Samples must be sent immediately to the laboratory.

ACCEPTANCE CRITERIA FOR VERBAL REQUESTS (REPEAT / *ADDITIONAL TEST)

1.  Request must be made on the same day sample is sent to the laboratory.
2.  Request can only be accepted if sample is sufficient.
3.  Requested test is suitable to sample collected.
4.  Request must be made by an authorized clinician.
5.  *Additional test is applicable for paediatric, geriatric or very ill patients.
