Rejection Criteria
9.1 First level rejection criteria - samples shall not be accepted or processed if any of the following criteria exist:
- No name on the request form / specimen.
- Requesting hospital / unit / ward / clinic not identified.
- Incomplete identification card / registration number.
- No name, signature and stamp of requesting doctors.
- No clinical history information.
- Information in request form and sample container is different.
- Mislabeling of sample / request form.
- Unsatisfactory specimen when received, for example spillages, breakages and etc.
- No hazardous sticker for infectious disease / sample.
9.2 Second level rejection criteria – sample not suitable to process if the following criteria exist:
- Inappropriate sample container / request form.
- Blood is haemolysed / clotted.
- Insufficient sample.
- Aged sample.
- Sample is not suitable for analysis.
- Test is not available.
- Inappropriate sample transportation.
