Acceptance Criteria for Verbal Requests (Tranfusion Med)

6.1 Verbal request can be made to add-on blood components, additional testing or to inform cancellation of test on specimen that previously submitted.

6.2 Immunohaematology Request

         6.2.1 Request should be made less than 24 hours after sample is received.

       6.2.2 Request can only be accepted if sample is sufficient.

6.2.3 Requested test is suitable to sample.

6.2.4 Request should be made by the doctor.

6.3 Blood/ Blood Component request

         6.3.1 Request should be made less than 48 hours after sample is received.

         6.3.2 New request form must be send to UPT after verbal request is done.

         6.3.3 If sample is lysed or inadequate new blood sample needed.
